Jeffrey Polk was born December 5, 1963 to the late Thelma Jean Polk. He accepted Christ as his personal Savior at an early age. 

His graveside service was held on Saturday January  29, 2022 at 11:00 am at the Mays Hill MB Church Cemetery at with Pastor Eddie L. Walton in charge. Dixie Funeral Home www.dixiefuneralhome.com was in charge of the professional service.

He leaves to cherish his memories one daughter, Alexis D. Polk; one stepson, Tony L. Hayes; one sister, Felicia Simmons; one brother, Terrance Wright; two aunts, Linda Polk and Vernell Polk; and a host of nieces, nephews, cousins and friends that loved him dearly.
